Hospital Records

A while ago I had requested a copy of ALL medical records on PooWee. I got them, though I believe they did not give me all of them. Here’s what I have discovered. (If you click on the picture you should beable to magnify areas.)
This is a hospital form. Notice the circled PROSPECTIVE PARENTS , MY BABY and MOTHER’S SIGNATURE. However, this is also the form that states what I am to recieve. It doesn’t say anything about PAPs getting any items. HHMMMM!
These two forms are basically the same. I’m either addressd as parent/legal guardian or patient. However at the lower left corner you will see one is for Newborn and the other states BIRTHMOTHER. I also get a “kick” out of the comment..Motivated by the compassion of Christ, we help people improve the quality of their lives.” WHERE WAS MY HELP??!! Mag did provide help AFTER I brought PooWee home, but nothing when I was in a crisis pregnancy. Unless you say, taking my baby away from me as help!
These two forms are The Legal Risk Placement forms from the Adoption agency. Their form addresses them PROPERLY as PROSPECTIVE ADOPTIVE PARENTS.
My form adresses me as BIRTHMOTHER!
This form is a discharge form from the hospital. I had to put it up because I applaud the nurse, you can see J signed by the parent signature spot. Wrong! So she had to resign at the bottom. I’m confused though, because I signed a discharge form. And then the nurse had J sign above my signature too. That form is not in with my records. Must have got tossed-ah?!
This next form is the consent for circumcision 
Here, you will see my error! Here, I addressed myself as BIRTHMOTHER. HOW UNEDUCATED AND STUPID I WAS!!!
Summary: The hospital was correct in their termanology of me-MOTHER. However, they neglected to get me my requested items. I do believe some forms are missing from this chart though. 1. the discharge form I signed and 2 the form J & P supposedly signed to take PooWee home in an expired carseat.
The adoption used INCORRECT termminology of me. I was addressed as BIRTHMOTHER severle times. I’m curious to know what other forms that I signed were like this. I wonder what forms I all signed at the adoption agency. HHMMM-do I ask for more records?

Looking at all of that made me want to throw up a little bit. I can’t imagine what any of my papers said. It’s all been blocked.

The tactics they used as referencing you to the terms birthmother pre-adoption is soo NCFA and just TASTELESS!! It makes that little bit of vomit come up your throat doesn’t it.
The fact that you have both is REALLY interesting. This in itself looks to me like a documented form of coersion. And you have the proof. Please hold onto those, don’t let them go anywhere.

Frankly, I do not see where any agency has the right to have you sign one damned thing before TPR.
That “Legal Risk Placement Agreement for BIRTHMOTHER”? Sickening. It is absolutely a form of coercion. I am literally reeling over here.

I really really really want copies of my paper at the adoption agency!!!!! The thing I struggle with is that I am on such good terms with Mag. I do love her. I want to ask her why these forms are this way. But, I don’t want to come across the wrong way. I don’t want her to think I’m “targeting” her, when it’s the organization I’m targeting. Ya know?
